FINAL REQUEST FOR SOCIAL INSURANCE NUMBER AND DATE OF BIRTH TO PROCESS PAYMENT FOR APS V. FLIGHT CENTRE TRAVEL GROUP (CANADA) INC. SETTLEMENT

Dear class member,

The Administration Form for the Aps. v Flight Centre Travel Group (Canada) Inc. class action required you to provide Trilogy Class Action Services (the “Claims Administrator”) with your Social Insurance Number and Date of Birth in order to process your payment. You failed to provide this information on your Administration Form. We have made multiple attempts to contact you to obtain this information, but have still not received this information from you.

Please provide the Claims Administrator with your Social insurance Number, Date of Birth and your Claim ID number by email to by no later than February 28, 2022.

The CRA charges a $100.00 charge for processing T4 forms which do not contain a Social Insurance Number. Please be advised that if you do not provide your Social Insurance Number to the Claims Administrator by February 28, 2022 this amount will be deducted from any amount that would otherwise be owing to you. If the amount owning less the $100.00 CRA charge is a positive balance then you will be paid the net amount. If the amount owing to you less the $100.00 CRA charge is a negative balance, then you will not be paid.
